Getting around McDonough
Getting around McDonough is an I-75 story with a square at the middle. The interstate carries the region's traffic past the city's four exits, GA 20 and GA 81 meet at the courthouse square, and US 23 / GA 42 runs the downtown one-way pair. Almost every McDonough trip starts on one of those.
Transit is a weekday commuter bus, Xpress Route 430, from the local park-and-rides into Atlanta. There is no MARTA rail in Henry County, and this page says the honest version of that plainly.

The interstate exits and the square highways
I-75, the four McDonough exits
Exit 216 (SR 155, McDonough Road), Exit 218 (SR 20 and SR 81, the main downtown exits), Exit 221 (Jonesboro Road), and Exit 222 (Jodeco Road) all serve the city, with Exit 224 at Eagles Landing just north in Stockbridge. I-75 is the airport and Atlanta corridor and the region's freight spine.
GA 20 and GA 81, at the square
Both state routes run through downtown and meet around the courthouse square, the traditional crossroads the city grew around. GA 20 runs east-west through the county, and GA 81 connects the north toward Stockbridge and Hampton.
US 23 / GA 42, the downtown one-way pair
The old highway runs through downtown as a one-way pair around the square, carrying the through traffic of the pre-interstate era and still the local's route through the historic district.
GA 155, McDonough Road
Meets US 23 / GA 42 just south of the Town Green rather than crossing the square, which is why a downtown bypass, the McDonough Parkway Extension, is being built to pull the through traffic off the historic center.

What the drives to the usual destinations actually take
Times below are published figures for normal, uncongested traffic on I-75. The weekday peaks change them considerably, and the airport run is the one everyone asks about first.
Hartsfield-Jackson airport
About 30 to 35 minutes
Roughly 23 to 24 miles up I-75, about 30 to 35 minutes in normal traffic. The interstate runs through the airport's back door, so the math is one road the whole way.
Downtown Atlanta
About 35 to 40 minutes
About 28 miles up I-75 to the downtown connectors. Off-peak it is a clean run; at rush hour, plan around the I-75 and I-285 merge.
Stockbridge and the north side
About 10 to 15 minutes
The next cities up the corridor, an everyday run for the Stockbridge retail and the Eagles Landing side of the county.

The honest transit picture: one weekday commuter bus
Henry County has no MARTA rail, and the honest first fact is that McDonough's transit is a single weekday story: Xpress Route 430 runs peak-hour commuter buses from the McDonough and Stockbridge park-and-rides into downtown and midtown Atlanta, a published ride of roughly 45 to 50 minutes, weekdays only, no weekends.
The airport has no direct bus from McDonough. A transit trip to Hartsfield-Jackson means driving to the airport or connecting through Atlanta, which for most households beats taking a commuter bus aimed at downtown and then riding rail back out. The honest recommendation for the airport run is the car.
On the ground, the square is walkable in the old center, and the everyday errands run along the GA 20, GA 81, and Jonesboro Road frontage that a car covers easily. For the commuter with a real downtown schedule, Route 430 from the park-and-ride works; for everyone else, I-75 is the transit system.
Schedules and fares change with the season. Confirm the current Xpress timetable before your first ride; the park-and-ride lots fill early on weekday mornings.
